EASTSIDE TEEN DRIVER'S EDUCATION

Registration Requirements

Parents or guardian must come in person to register. A minimun deposit of $50.00 is required.
A second payment is due on the 2nd week, and final payment is due before the first drive.

D.P.S REQUIREMENTS FOR PERMIT:

*STUDENT MUST BE 15YEARS OF AGE ANS WILL NEED TO THE FOLLOWING DOCUMENTS:
(1) Original State Certifed Birth Ceritificate or green card
(2) DE-964 (Driving school will provide
(3) Verification of enrollment form from your high school
(4) Social Secuirty Card (original)
(5) One Parent
(6) DPS Application plus Fee(s)
(7) Plus one of the following: TX ID Card, Shot Record, School ID, Military ID, or Health Insurance Card with students name.

D.P.S REQUIREMENTS FOR LICENSE:

* STUDENT MUST BE 16YRS OF AGE AND HAVE HELD PERMIT AT LEAST 6 MONTHS AND WILL NEED THE FOLLOWING DOCUMENTS:
(1) DE-964
(2) Verification of enrollment form from your high school
(3) Permit
(4) $10.00

***Remember to give your Insurance card company a copy of your DE-964 for a 10% discount. We charge for duplicates.
Please keep original DE-964 for your records. We charge for duplicates or copies.
